Digital tattoo health and consent form

A digital tattoo health form (anamnesis) is the health and consent form the client fills in and signs on a phone or computer before the session. In Kayru, the artist sends a link; answers, CPF and signature are stored encrypted, with date, time, IP and device recorded, and the form becomes a PDF whenever the artist needs it.

How it works, step by step

When you book a session, Kayru emails the form link if the client has an email on file. If not, you open WhatsApp with the link ready and send the message yourself.

If the form is still unsigned, the email is sent again 2 days before the session. Once the client signs, the form shows up in their file.

What the client fills in

Health questions (medical treatment, allergies, diabetes, healing problems, pregnancy and others), CPF, date of birth and image-use authorization. Then they read the consent term and sign, either by drawing their signature or typing their name.

Only adults (18+) can sign the electronic form.

Security and LGPD (Brazil's data protection law)

Answers, CPF and signature are stored encrypted. Health data is sensitive data under the LGPD (art. 11), and the form records the signature trail: date and time, IP, device, term version and an integrity hash.

You are the controller of your clients' data; Kayru (Eikon) is the processor, storing and handling the form on your behalf.

PDF and authenticity check

Every signed form becomes a PDF on demand, straight from the client's file, with a final page that records the signature and carries a verification code.

Anyone can check the PDF's authenticity on a public page using the code. If the client types their email on the form, they get a signed copy.

How long the form is kept

A signed form is kept for 5 years from signature, even if the client is deleted, for legal defense if the work is ever questioned. You can download the PDFs while your space exists and for 12 months after your subscription ends.

Paper or digital

On paper, the form depends on a drawer, legible handwriting and someone remembering where it went. Digital, it lives in the client's file, with a record of who signed, when and which term was accepted.

The form records evidence; it is not legal advice. For a specific case, talk to a lawyer.

Frequently asked questions

Is a digital tattoo health form valid?

Kayru's form uses a simple electronic signature and records evidence: who signed, when, from which device and which term version was accepted, with an integrity hash. It is not an ICP-Brasil certificate. For how it applies to a specific case, consult a lawyer.

What questions should a tattoo health form include?

The ones that affect the session and healing. Kayru's form asks about medical treatment, allergies, diabetes, hemophilia, healing problems, keloids, pregnancy, breastfeeding, blood pressure, heart disease and hepatitis, among others, plus CPF, date of birth and image use.

Can a minor sign the health form?

Not through Kayru's electronic form: only adults (18+) can sign it.

Who is responsible for health form data under the LGPD?

The tattoo artist is the controller of client data; Kayru (Eikon) is the processor. Health data is sensitive data (LGPD art. 11), so answers, CPF and signature are stored encrypted.
